Diabetes Care Center
Helping you manage diabetes for a long, fulfilling life.
Diabetes is a disease that requires lifetime management. Whether you have had diabetes for many years or have recently been diagnosed, the Diabetes Care Center of Johnson Memorial Hospital is here to help.
Education about diabetes self-management techniques helps you learn the skills needed to balance all aspects of self-care: medications, eating habits, exercise, and blood glucose monitoring. Our staff works with you to develop an individual plan to meet your lifestyle needs. A comprehensive education class series is available, as well as appointments that focus on specific self-care practices.
Recognized by the American Diabetes Association
In early 2009 the American Diabetes Association renewed the Center's Program Recognition status, assuring patients that the education provided at the Diabetes Care Center meets National Standards for Diabetes Self-Management Education criteria. The ADA’s recognition of the Center’s program is required for the Center to continue to participate in the Medicare and Medicaid reimbursement programs and is a sign of high quality education services.
